10/28/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Kaila upon arrival to the home setting. Upon arrival to the home setting, Kaila appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Intervention QMHPA began the session by inquiring about Kailaâs current thoughts, feelings and emotions. QMHPA assisted Kaila on confidence building by helping her with independent living skills. QMHPA engaged Kaila in a skills building activity. QMHPA explained to Kaila that the purpose of this activity was to discuss with Kaila the importance of being able to complete daily needs without any prompts. QMHPA demonstrated with Kaila appropriate time management skills by completing a daily log of activities and chores. QMHPA provided Kaila with literature on hygiene regiments. QMHPA engaged Kaila in another skills building activity. QMHPA explained to Kaila that the purpose of this activity was to educate Kaila on the importance of showering, wearing clean clothes, and completing chores. QMHPA reviewed with Kaila his log of chores and assisted her with identifying the things her didn't complete. QMHPA educated Kaila on appropriate budgeting techniques. QMHPA assisted Kaila with identifying bulk goods that he can purchase so it will last longer. Response: Kaila appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Kaila said he was able to understand independent living skills. Kaila does take showers but she will put the same clothes back on. Kaila stated she don't know why she put on the same clothes back on. Kaila does not follow proper hygiene regiments such as washing her hands daily, wiping her hands on her shirt. Kaila was able to understand some bulk items she like and said she will purchase bulk products when she gets her check. She indicated to QMHPA during the skills building activity that she thinks that she is getting better with some things that QMHPA has been working with her on but she still have a long way to go. Plan: QMHPA will continue to support Kaila in identifying her daily needs and develop a chart so she can learn how to manage her funds successfully (1-2 times weekly).
